Student Financial Responsibility Agreement
This online form indicates your agreement to the terms and conditions of your St. Kate's student billing. This form must be completed once per academic career by all students prior to the start of attendance.
Title IV Federal Financial Aid Student Authorization
This form indicates your agreement to the terms of applying Title IV Federal Financial Aid funds to pay for charges other than tuition, fees, room, and board. Some examples of other charges include: parking permits, parking fines, bookstore charges, health insurance, health and wellness charges. Once in Katepay, click My Account in the top right corner and go to Consents and Agreements. Once there, you'll be able to complete the Title IV Federal Financial Aid Student Authorization Form.
Immunization Verification
Minnesota requires that students born after 1956 be immunized against tetanus, diphtheria, measles, mumps and rubella, allowing for certain specified exemptions. You must only complete this form once. You must be registered for classes to access the forms.
Immunization forms will be available in August.
Health Insurance Waiver
St. °µÍø½ûÇø provides health insurance for the students in the College for Women, OTA Online and the College for Adults—bachelor's and associate programs and OTA. The University does not provide health insurance to graduate students, but coverage is required of all students in the Henrietta Schmoll School of Health for clinical education purposes.
St. Kate's Financial Aid Application
To be considered for state and institutional financial aid, you must also submit the St. Kate's Financial Aid Application (which is separate from the FAFSA). If you are a College for Women, first-time first-year student you are not required to complete the St. Kate's Financial Aid Application.
